Surgical procedure with IntraGraft® FAT

    1 | INFILTRATION

    Infiltration is performed by injecting 60cc of Klein solution (normal saline – adrenaline – lidocaine) into the donor site to enable a safe and painless harvest of autologous adipose tissue.

    The Tumescence Infiltration Cannula features small, multiperforated openings strategically oriented to allow uniform 360-degree distribution of the solution throughout the donor area.

    2 | FAT HARVESTING

    Using dedicated Carraway cannulas, 15–20cc of subcutaneous fat is harvested.
    This adipose tissue is essential for the preparation of NanoFat and MicroFat, both of which are key components in regenerative medicine applications.
    The cannula features non-linear port arrangements near its tip, designed to optimize fat collection.
    The syringes are equipped with vacuum lock systems to facilitate and accelerate the harvesting process.

    3 | GRAVITY DECANTATION (3–5 min)

    Use the Nanofat filter as a support for the harvesting syringe.
    The adipose tissue is allowed to decant in order to separate unwanted fractions and isolate the functional layer, which is rich in mesenchymal stem cells.
    This process eliminates the need for centrifugation, preserving cell viability.

    Layer Composition:

    • Lipid Layer (Supernatant): Oily upper layer
    • Adipose Tissue: Middle golden layer — desired fraction
    • Fluid Layer (Infranatant): Lower red layer — contains blood and tumescent solution (undesired)

    Steps:

    • Remove any excess air before starting decantation
    • Use the Nanofat filter as a support during the process
    • Let the adipose tissue decant by gravity in the syringe for 3–5 minutes
    • Expel the infranatant (fluid below the fat) after decantation

    4 | PROGRESSIVE FILTERING

    The tissue is progressively micronized until NanoFat is obtained, a fluid and regenerative suspension. This material is particularly indicated for osteoarthritis, meniscal and cartilage lesions, tendinopathies, muscle injuries and volume loss, and neuropathies.
    Transfer the decanted adipose tissue into a single 20cc syringe using the GEMS Anaerobic Transfer 2.4 mm.

    Luer-to-Luer:
    Sizes: 2.4 mm (purple), 1.4 mm (blue), and 1.2 mm (green).
    → Preserves the anaerobic and closed environment of the grafts during sizing.
    Designed to produce micronized fat of progressively smaller sizes by passing the tissue back and forth through each transfer 30 times.

    Final filtering NanoFat 400nm
    Take the syringe containing the previously emulsified adipose tissue, connected to the Nano Transfer at the top, and connect an empty syringe on the side.
    Perform a single transfer of the material to obtain the NanoFat (TNF).

    5 | INJECTION

    NanoFat is easily injectable with very fine needles (27G or 30G), allowing both superficial and deep treatments, making it ideal for applications in Sports Medicine as well.
